ABSTRACT

The role of white matter of brain has always been neglected by scholars.With the development of neuroimaging technology,the role of white matter has attracted increasing attention.Perioperative neurocognitive disorders have been a hot issue in the research on anesthesia,and recent studies have suggested that white matter may be involved in the effects of general anesthetics on cognitive function.This paper reviews the progress in the relationship between white matter,general anesthesia,and cognitive function from clinical practice and research,aiming to provide new ideas for the research on the mechanism.

ABSTRACT

Diabetic wounds are a common complication of diabetic patients, and the incidence has been increasing in recent years. In addition, its poor clinical prognosis seriously affects the quality of life of patients, which has become the focus and difficulty of diabetes treatment. As the RNA regulating gene expression, non-coding RNA can regulate the pathophysiological process of diseases, and play an important role in the healing process of diabetic wounds. In this paper, we reviewed the regulatory role, diagnostic value, and therapeutic potential of three common non-coding RNA in diabetic wounds, in order to provide a new solution for the diagnosis and treatment of diabetic wounds at the genetic and molecular level.

ABSTRACT

ObjectiveTo investigate the therapeutic effect of the frozen and fresh preparations of human umbilical cord mesenchymal stem cells (hUC-MSC) on a rat model of liver cirrhosis after transplantation via the portal vein or the caudal vein. MethodsA total of 70 specific pathogen-free healthy male Sprague-Dawley rats were randomly divided into normal group (13 rats fed with ordinary tap water and rat food) and liver cirrhosis model group (57 rats given subcutaneous multi-point injection of mixed carbon tetrachloride/olive oil solution). At week 8, the growth of rats was observed for both groups, and 3 rats were selected from each group for histopathological examination to confirm the formation of liver cirrhosis. A total of 50 rats were selected from the liver cirrhosis model and were divided into model group, portal vein group+fresh cell preparation group, portal vein+frozen cell preparation group, caudal vein+fresh cell preparation group, and caudal vein+frozen cell preparation group using a random number table, with 10 rats in each group. Fresh or frozen hUC-MSC were transplanted via the portal vein or the caudal vein, and after 4 weeks of administration, the different groups were compared in terms of the changes in liver function parameters and liver fibrosis degree. Continuous data were expressed as mean±standard deviation, and the independent-samples t test was used for comparison between two groups; a one-way analysis of variance was used for comparison between multiple groups, and the least significant difference t-test was used for further comparison between two groups. ResultsAt week 8 of modeling, the model group showed the formation of pseudolobules of different sizes in the liver and met the diagnostic criteria for liver cirrhosis, with significant increases in the levels of alanine aminotransferase (ALT), aspartate aminotransferase (AST), total bilirubin (TBil), and alkaline phosphatase (ALP) compared with the normal group (all P<0.001), suggesting that the rat model of liver cirrhosis was established successfully. There were significant differences in the levels of ALT, AST, TBil, and ALP between the five groups (F=232.00, 177.10, 112.30, 121.70, all P<0.001). Further comparison between two groups showed that the model group had significantly higher levels of ALT, AST, TBil, and ALP than the normal group (all P<0.01), and the portal vein group+fresh cell preparation group, the portal vein+frozen cell preparation group, the caudal vein+fresh cell preparation group, and the caudal vein+frozen cell preparation group had significantly lower levels of ALT, AST, TBil, and ALP than the model group (all P<0.01). ConclusionThere are significant improvements in liver function and liver fibrosis degree in a rat model of liver cirrhosis at week 4 after the transplantation of hUC-MSC, and frozen or fresh cell preparation and different transplantation approaches have no significant influence on treatment outcome.

ABSTRACT

ObjectiveTo investigate the effect of mirror therapy with augmented reality on attention of stroke patients. MethodsFrom January, 2020 to December, 2022, 60 stroke patients in the First People's Hospital of Changzhou were randomly divided into control group (n = 30) and observation group (n = 30). Both groups received routine occupational therapy, and the observation group received mirror therapy with augmented reality additionally, for four weeks. They were assessed with Digit Span Test (DST), Digit Cancellation Test (D-CAT3 and D-CAT3P), Symbol Digit Modalities Test (SDMT) and Paced Auditory Serial Addition Test (PASAT) before and after treatment. ResultsBefore treatment, there was no significant difference in the scores of the DST, D-CAT3, D-CAT3P, SDMT and PASAT between two groups (P > 0.05). After treatment, all the indexes improved in the observation group (|t| > 3.663, P < 0.01), and improved more in the observation group than in the control group (|t| > 2.037, P < 0.05). ConclusionThe mirror therapy with augmented reality could effectively improve attention of stroke patients in the short term.

ABSTRACT

Objective: To explore the value of cerebral hypoxic-ischemic injury markers in the early diagnosis of sepsis associated encephalopathy (SAE) in burn patients with sepsis. Methods: A retrospective case series study was conducted. From October 2018 to May 2021, 41 burn patients with sepsis who were admitted to Zhengzhou First People's Hospital met the inclusion criteria, including 23 males and 18 females, aged 18-65 (35±3) years. According to whether SAE occurred during hospitalization, the patients were divided into SAE group (21 cases) and non-SAE group (20 cases). The gender, age, deep partial-thickness burn area, full-thickness burn area, and acute physiology and chronic health evaluation Ⅱ (APACHE Ⅱ) scores of patients were compared between the two groups. The serum levels of central nervous system specific protein S100β and neuron specific enolase (NSE) at 12, 24, and 48 h after sepsis diagnosis (hereinafter referred to as after diagnosis), the serum levels of interleukin-6 (IL-6), IL-10, tumor necrosis factor α (TNF-α), Tau protein, adrenocorticotropic hormone (ACTH), and cortisol at 12, 24, 48, 72, 120, and 168 h after diagnosis, and the mean blood flow velocity of middle cerebral artery (VmMCA), pulsatility index, and cerebral blood flow index (CBFi) on 1, 3, and 7 d after diagnosis of patients in the two groups were counted. Data were statistically analyzed with chi-square test, analysis of variance for repeated measurement, independent sample t test, and Bonferroni correction. The independent variables to predict the occurrence of SAE was screened by multi-factor logistic regression analysis. The receiver operating characteristic (ROC) curve was drawn for predicting the occurrence of SAE in burn patients with sepsis, and the area under the curve (AUC), the best threshold, and the sensitivity and specificity under the best threshold were calculated. Results: The gender, age, deep partial-thickness burn area, full-thickness burn area, and APACHE Ⅱ score of patients in the two groups were all similar (χ2=0.02, with t values of 0.71, 1.59, 0.91, and 1.07, respectively, P>0.05). At 12, 24, and 48 h after diagnosis, the serum levels of S100β and NSE of patients in SAE group were all significantly higher than those in non-SAE group (with t values of 37.74, 77.84, 44.16, 22.51, 38.76, and 29.31, respectively, P<0.01). At 12, 24, 48, 72, 120, and 168 h after diagnosis, the serum levels of IL-10, Tau protein, and ACTH of patients in SAE group were all significantly higher than those in non-SAE group (with t values of 10.68, 13.50, 10.59, 8.09, 7.17, 4.71, 5.51, 3.20, 3.61, 3.58, 3.28, 4.21, 5.91, 5.66, 4.98, 4.69, 4.78, and 2.97, respectively, P<0.01). At 12, 24, 48, 72, and 120 h after diagnosis, the serum levels of IL-6 and TNF-α of patients in SAE group were all significantly higher than those in non-SAE group (with t values of 8.56, 7.32, 2.08, 2.53, 3.37, 4.44, 5.36, 5.35, 6.85, and 5.15, respectively, P<0.05 or P<0.01). At 12, 24, and 48 h after diagnosis, the serum level of cortisol of patients in SAE group was significantly higher than that in non-SAE group (with t values of 5.44, 5.46, and 3.55, respectively, P<0.01). On 1 d after diagnosis, the VmMCA and CBFi of patients in SAE group were significantly lower than those in non-SAE group (with t values of 2.94 and 2.67, respectively, P<0.05). On 1, 3, and 7 d after diagnosis, the pulsatile index of patients in SAE group was significantly higher than that in non-SAE group (with t values of 2.56, 3.20, and 3.12, respectively, P<0.05 or P<0.01). Serum IL-6 at 12 h after diagnosis, serum Tau protein at 24 h after diagnosis, serum ACTH at 24 h after diagnosis, and serum cortisol at 24 h after diagnosis were the independent risk factors for SAE complicated in burn patients with sepsis (with odds ratios of 2.42, 1.38, 4.29, and 4.19, 95% confidence interval of 1.76-3.82, 1.06-2.45, 1.37-6.68, and 3.32-8.79, respectively, P<0.01). For 41 burn patients with sepsis, the AUC of ROC of serum IL-6 at 12 h after diagnosis for predicting SAE was 0.92 (95% confidence interval was 0.84-1.00), the best threshold was 157 pg/mL, the sensitivity was 81%, and the specificity was 89%. The AUC of ROC of serum Tau protein at 24 h after diagnosis for predicting SAE was 0.92 (95% confidence interval was 0.82-1.00), the best threshold was 6.4 pg/mL, the sensitivity was 97%, and the specificity was 99%. The AUC of ROC of serum ACTH at 24 h after diagnosis for predicting SAE was 0.96 (95% confidence interval was 0.89-1.00), the best threshold was 14.7 pg/mL, the sensitivity was 90%, and the specificity was 94%. The AUC of ROC of serum cortisol at 24 h after diagnosis for predicting SAE was 0.93 (95% confidence interval was 0.86-1.00), the best threshold was 89 nmol/L, the sensitivity was 94%, and the specificity was 97%. Conclusions: Serum Tau protein, ACTH, and cortisol have high clinical diagnostic value for SAE complicated in burn patients with sepsis.

ABSTRACT

Objective: To explore the effects of expanded frontal-parietal pedicled flap in reconstructing cervical scar contracture deformity in children after burns. Methods: A retrospective observational study was conducted. From January 2015 to December 2020, 18 male children with cervical scar contracture deformity after burns who met the inclusion criteria were admitted to Zhengzhou First People's Hospital, aged 4 to 12 years, including 10 cases with degree Ⅱ cervical scar contracture deformity and 8 cases with degree Ⅲ scar contracture deformity, and were all reconstructed with expanded frontal-parietal pedicled flap. The surgery was performed in 3 stages. In the first stage, a cylindrical skin and soft tissue expander (hereinafter referred to as expander) with rated capacity of 300 to 500 mL was placed in the frontal-parietal region. The expansion time was 4 to 6 months with the total normal saline injection volume being 2.1 to 3.0 times of the rated capacity of expander. In the second stage, expander removal, scar excision, contracture release, and flap transfer were performed, with the flap areas of 18 cm×9 cm to 23 cm×13 cm and the secondary wound areas of 16 cm×8 cm to 21 cm×11 cm after scar excision and contracture release. After 3 to 4 weeks, in the third stage, the flap pedicle was cut off and restored. The rated volume of placed expander, total normal saline injection volume, type of vascular pedicle of flap, survival of flap and reconstruction of scar after the second stage surgery were recorded. The neck range of motion and cervico-mental angle were measured before surgery and one-year after surgery. The appearance of neck, occurrence of common complications in the donor and recipient sites of children, and satisfaction of children's families for treatment effects were followed up. Data were statistically analyzed with paired sample t test. Results: All the patients successfully completed the three stages of operation. The rated volume of implanted expander was 300 mL in 6 children, 400 mL in 9 children, and 500 mL in 3 children, with the volume of normal saline injection being 630 to 1 500 mL. The type of vascular pedicle of flap was double pedicle in 13 cases and was single pedicle in 5 cases. All the flaps in 17 children survived well, and the secondary wounds after neck scar excision and contracture release were all reconstructed in one procedure. In one case, the distal blood supply of the single pedicled flap was poor after the second stage surgery, with necrosis of about 2.5 cm in length. The distal necrotic tissue was removed on 10 days after the operation, and the wound was completely closed after the flap was repositioned. In the follow-up of 6 months to 3 years post operation, the cervical scar contracture deformity in 18 children was corrected without recurrence. The flap was not bloated, the texture was soft, and the appearances of chin and neck were good. The range of motion of cervical pre-buckling, extension, left flexion, and right flexion, and cervico-mental angle in one year after operation were improved compared with those before operation (with t values of 43.10, 22.64, 27.96, 20.59, and 88.42, respectively, P<0.01). The incision in the frontal donor site was located in the hairline, the scar was slight and concealed. No complication such as cranial depression was observed in expander placement site, and the children's families were satisfied with the result of reconstruction. Conclusions: Application of expanded frontal-parietal pedicled flap in reconstructing the cervical scar contracture deformity in children after burns can obviously improve the appearance and function of neck, with unlikely recurrence of postoperative scar contractures, thus it is an ideal method of reconstruction.

ABSTRACT

As a new form of medical industry, internet medicine has not only the endogenous risk of internet industry, but also the blind area and exogenous risk because of lagging regulation. With the continuous progress of internet information technology, online diagnosis and treatment will gradually integrate and reconstruct the traditional medical service. In the history of medical development, the emergence of new technologies generally has the characteristics of risk. Therefore, the supervision of internet medicine should adhere to the rule of inclusiveness and prudence, and take into account the cultivation of the market and standardized development. It should also build a regulatory community, coordinate governance from the four dimensions of legitimacy, compliance, rationality and superiority. Taking the law as the bottom line, we should make full use of early online warning monitoring, off-site supervision, online supervision, reputation mechanism and other measures to promote the innovative development of internet medicine under the framework of legal compliance.

ABSTRACT

The penetration and integration of internet with medical industry is apparently falling behind its interaction with other industries in terms of speed and scale. The main reason is due to the high risk and difficult supervision of internet medicine. Based on the regulatory context, this paper analyzes the endogenous and exogenous risk of internet medicine. The inherent characteristics of the internet (such as openness, diversity, multi subjectivity, virtuality, transparency, and high aggregation) lead to endogenous risks, including jurisdiction disputes, uncertainty of rights and obligations, security of personal information and medical information, monopoly. The lag of specific law in internet medicine, the lack of extensibility and synergy of traditional hierarchical regulatory system, the uncertainty of internet hospital regulatory policy details, the lack of technical specifications/operating procedures and other external system coverage in internet diagnosis and treatment lead to regulatory blind areas and exogenous risks. Based on the risk analysis, this paper discusses further construction of the mechanisms of risk assessment and regulation for internet medicine.

ABSTRACT

Internet medicine is not a concept of jurisprudence. China's current medical law system mainly focuses on main qualifications of the participants who conduct diagnosis and treatment. This paper focuses on several business models of the core element (diagnosis and treatment) in internet medicine, namely: online health consultation provided by internet medical platform, online diagnosis and treatment by internet hospital, and certain artificial intelligence(AI) medicine products that are specialized for diagnosis and treatment. This paper summarizes the development of these three business models, and analyzes the main problems in their development, including the legitimacy identification of online consultation, the dilemma of the development of internet hospitals, the impact of insufficient sample data on the development of medical AI, and the reverse restriction of regulatory difficulties to the development of internet medicine. The next step of developing internet medicine is to broaden the scope of internet diagnosis and treatment on the premise of medical safety, which will be based on the improvement of current regulatory system. It is urgent to build early risk warning system and supervision/ management mechanism for internet medicine.

ABSTRACT

In the fifth scientific and technological revolution, information technology is the first productivity, which has a great impact on the supply and demand of medical services. Generally, internet medicine is equivalent to the combination between health industry and information technology. "Healthy China" strategy is China's "priority development strategy", which adheres to the principle of health equity, emphasizes the integration of health into all policies, and takes co-construction and sharing as the basic path. With implementing internet medicine, "Healthy China" strategy promotes the mobility of medical service with three tools: interconnection, data and artificial intelligence. This enhances the operation efficiency of overall medical and health system, and optimizes the allocation of medical resources. The future development of internet medicine follows the double helix mode driven by technology and policy, and the policy determines the development boundary of the industry. On the basis to ensure medical safety, we should explore the possibility of internet diagnosis and treatment further, and pay attention to the fairness of resource allocation while improving efficiency, so as to realize the co-construction and sharing of health services.

OBJECTIVE@#To explore the risk factors of detection of uric acid crystals by dual energy CT (DECT) in patients with gout diagnosed by gold standard.@*METHODS@#From June 2011 to December 2018, clinical data of 29 patients were collected who were diagnosed with acute or chronic gout by positive polarized light analysis of joint synovial fluid in First Hospital of Peking University. Chi-square test, Logistic regression and t-test were used. The relationship between DECT and the clinical data, laboratory examination and drug treatment were analyzed.@*RESULTS@#In this study, 29 patients were included, of whom, 22 patients were detected with uric acid crystals by DECT, and 7 patients were not. According to whether the uric acid crystals were detected or not by DECT, the patients were divided into two groups. Compared with the negative group, the patients were older in positive group [(47±12) vs. (39±11) years, P=0.15], had higher body bass index (BMI) [(27.9±3.7) vs. (22.8±2.1) kg/m2, P=0.002], longer gout disease duration [(135±102) vs.(45±53) months, P=0.035], higher in the highest serum uric acid in history [(643±121) vs. (543±103) μmol/L, P=0.043]. Although uric acid near DECT in positive group was higher than in negative group, there was no statistical difference [(558±150) vs. (513±89) μmol/L, P=0.497]. Comparing positive group with negative group, the percentage of the patients in acute phase was higher than in chronic phase [18(81.8%) vs. 4(57%), P=0.311];the percentage of the patients taking uric-acid-lowering drugs was higher than the other group [22(100%) vs. 5 (71%), P=0.052];the percentage of the patients with recurrent typical attacks was higher than that of those without typical attacks [22 (100%) vs.6 (85%), P=0.241]. The consistency of symptoms and the finding of uric acid crystals by DECT had been compared between the joints. The right knee joint had the highest consistency (Kappa=0.627), followed by the left MTP1 (Kappa=0.58), the right metatarsophalangeal 1(MTP1, Kappa=0.551) and the left knee (Kappa=0.494), all of which had statistical significance. The consistency of the ankle joint was lower (the right ankle joint: Kappa=0.19, the left ankle joint: Kappa=0.256), showing no statistical significance. BMI (kg/m2) [2.307 (1.139-4.670), P=0.02], gout duration (years) [0.306 (0.906-4.881), P=0.186], and the highest uric acid level in history (mg/dL) [0.023 (0.981-2.764), P=0.137] had relationship to the positive result of urate crystals in DECT.@*CONCLUSION@#Gout patients with larger BMI, higher previous highest uric acid value and longer gout duration had higher sensitivity of the positive result in DECT.

ABSTRACT

Objective To investigate the effect of compound Ziyin granule (CZG) on sex hormones and oxidative stress levels of ovariectomized rats. Methods The experiment was divided into model group, Liuwei Dihuang pill group, estradiol valerate group, Zishen Yangyin Decoction group, CZG (low, medium, high dose) group, ormal group and sham operation group. After administration for 8 weeks, serum estradiol (E2), follicle stimulation hormone (FSH), luteotropic hormone (LH), total-superoxide dismutase (T-SOD), malonaldehyde (MDA) and glutathione peroxidase (GSH-Px) were measured. Results Compared with the model group, E2 was significantly raised at medium and high doses of CZG, LH was decreased at medium dose, and FSH was reduced at low, medium and high dose; medium and high dose of CZG were capable of remarkably increasing T-SOD, GSH-Px and decreasing MDA, and low dose raised T-SOD and reduced MDA (P<0.05 or P<0.01). Compared with estradiol valerate group, low dose of CZG significantly increased T-SOD (P<0.05). Compared with decoction group, GSH-Px was remarkably raised at medium and high dose (P<0.05). Conclusion The CZG could reverse the abnormal function of sex hormone secretion in the pituitary-ovary axis of the ovariectomy perimenopausal model rat and improve its antioxidant capacity.

ABSTRACT

Aim To study whether GLGZD exerts brain protection by affecting the activation of cortical microglia in cerebral ischemia-reperfusion rats. Methods The nylon thread plug was used to establish the MCAO model. After GLGZD treatment for seven days, mNSS was used to evaluate the neurological function of each group of rats, MRI to detect cerebral infarction volume in rats, TUNEL to detect the apoptotic rate of nerve cells, immunohistochemistry to detect TNF-α protein expression in ischemic cortical brain tissues, and RT-qPCR to detect mRNA expression of neuron-microglia interaction-related factors TWEAK, Fnl4, NIK, Rel B, CCL21, CXCR3 and microglial activation marker IBA-1 in ischemic cortical brain tissues. Results GL-GZD could significantly improve the neurological function of MCAO rats, and markedly reduce the infarct volume and apoptosis of ischemic cortical neurons in MCAO rats. It also could significantly down-regulate the expressions of TNF-a protein and TWEAK, Fnl4, NIK, Rel B, CCL21, CXCR3 and IBA-1 mRNA in ischemic cortex of MCAO rats. Conclusions GLGZD can significantly improve cerebral ischemia-reperfusion injury in rats, which may be related to inhibition of microglial cell activation by affecting TWEAK/Fn14/ CCL21/CXCR3 signaling pathway.

ABSTRACT

Objective To investigate the adults smoking status of Hui and Han in Ningxia. Methods Among 6 monitoring sites in Ningxia from 2013 to 2014, multi-stage stratified cluster random sampling method was used, information about risk factors of chronic diseases by were collected questionnaire investigation, a total of 3 527 subjects were effective sample for smoking analysis among 3 540 adults aged 18 and over. After the complex weighting, the smoking status of different nationalities, sexes and age was analyzed. Results The current smoking rate was 29.42% (95% CI:27.91%-30.92%), the current smoking rate of male(56.73%, 95% CI:54.25%-59.20%) was higher than female (0.92%, 95% CI:0.50%-1.34%) (P<0.001); The current smoking rate of Han (32.04%, 95% CI:30.24%-33.85%) was higher than Hui (23.09%, 95% CI:20.41%-25.76%). The daily smoking rate of Han (27.98%, 95% CI: 26.25%-29.72%) was higher than that of Hui nationality (19.83%, 95% CI:17.30%-22.36%) (P=0.001), and that of male (49.41%, 95% CI: 46.92%-51.91%) was higher than that of female (0.73%, 95% CI: 0.36%-1.11%) (P<0.001). The average daily smoking of male (20) was higher than that of female (8) (Z=-4.448, P<0.001). Smokers quit smoking rate was 12.54% (95% CI:10.50%-14.57%). Adult secondhand smoke exposure rate was 54.44% (95% CI:52.53%-56.36%). Conclusions The smoking rate of adult residents is high in Ningxia province, but the quit smoking rate is low, male smoking rate is higher than female, Han smoking rates higher than the Hui. The sample population was highly exposed to secondhand smoke. Tobacco control interventions should be taken against high-risk groups.

ABSTRACT

Objective:To observe the performance of the traumatic brain injury (TBI) patients with normal results in Mini-Mental State Examination (MMSE) in the Montreal Cognitive Assessment (MoCA) and Rivermead Behavioral Memory Test-3 (RBMT-3). Methods:From March, 2015 to June, 2018,40 TBI patients with cognitive complaints but normally in MMSE and other 30 healthy persons (controls) were assessed with MoCA and RBMT-3. Results:There were 14 patients sounded normally in MoCA. The scores of MoCA, as well as both the raw score and scale score of RBMT-3, were less in the patients than in the controls (t > 2.822, P < 0.01), especially in the verbal and delayed recall subtests of MoCA (t > 2.790, P < 0.01), and delayed story recall, skill learning instant recall, delayed skills recall (both raw and scale scores) (t > 2.507, P < 0.05), and remember name delay recall and story instant recall (raw score only) (t > 2.018, P < 0.05) of RBMT-3. Conclusion:For TBI patients with cognitive complaints, it is necessary to assesse with both MMSE and MoCA. If there is any memory impairment, RBMT-3 is needed.

ABSTRACT

Objective: To optimize the optional extraction process parameters of Modified Simiao Granule (MSG). Methods: HPLC was employed to determine the content of rhodoglycoside, chlorogenic acid, ammonium glycyrrhizate, berberine hydrochloride, and glycyrrhizin, using the extraction ratio, dried cream yield, and fingerprint similarity of which as comprehensive evaluation indexes, orthogonal test was used to investigate effects of solid-liquid ratio, extraction time and times on extraction process. Information entropy method was used to determine the objective weight of each index to achieve orthogonal test optimization of extraction process. Results: According to the result of comprehensive score, the optimal extraction conditions of this preparation were as follows: extracted twice with 10 times of water quantity, each for 30 min. The mean of the three batches of comprehensive evaluation was 88.91 and the RSD value was 0.61%. Conclusion: The optimized technologies are stable, repeatable, feasible and suitable for the industrial production of MSG.

ABSTRACT

AIM To establish an HPLC method for the simultaneous content determination of five constituents in Baishao Formula Granules (Paeoniae Radix Alba).METHODS The analysis of aqueous extract of this drug was performed on a 30 ℃ thermostatic Phenomenex C18 column (4.6 mm × 250 mm,5 μm),with the mobile phase comprising of acetonitrile-0.05% phosphoric acid flowing at 1.0 mL/min in a gradient elution manner,and the detection wavelength was set at 230 nm.RESULTS Paeoniflorin,albiflorin,paeoniflorin sulfonate,gallic acid and benzoic acid showed good linear relationships within the ranges of 0.020-0.639 mg/mL (r =0.999 8),0.005-0.172 mg/mL (r =0.999 9),0.020-0.652 mg/mL (r =1.000 0),0.003-0.097 mg/mL (r =0.999 8),0.002-0.058 mg/mL (r =0.999 7),whose average recoveries were 99.1%,98.3%,98.6%,98.1% and 99.5% with the RSDs of 1.86%,1.37%,1.69%,1.46% and 2.26%,respectively.The contents of various constituents in twenty-seven batches of samples demonstrated obvious differences.CONCLUSION We should pay attention to Paeoniae Radix Alba in Baishao Formula Granules due to its unstable quality.

ABSTRACT

Objective To study the effects of TGFβ-activated kinase-1 (TAK1)mediated cell autophagy after global cerebral ischemia-reperfusion (IR ) in rats.Methods Seventy-two male Kunming rats were randomly divided into six groups:control group (group C),sham operation group (group S),ischemia-reperfusion group (group IR),TAK1 shRNA lentivirus group (group T),nega-tive lentivirus group (group Y)and normal saline group (group NS)(n = 12 each).The rats in groups T,Y and NS received cerebral ventricles injection of TAK1 shRNA lentivirus,negative lenti-virus and normal saline 10 μl two weeks before preparing animal model.Using thread embolism of the right middle cerebral artery occlusion (MCAO)to cause focal ischemia for 2 h and released for 24 h for reperfusion in groups IR,T,Y and NS.The common carotid arteries were separated but not liga-ted in group S,the rest of the procedure as the same as group IR.The rats of each group were evalua-ted by neurological severity scores (NSS)24 h after reperfusion,the cerebral infarct volume was measured with the method of TTC and the expression of TAK1,LC3Ⅱ/LC3Ⅰ,Beclin1 and p62 pro-tein in rat hippocampus were determined by using Western blot.Results The infarct volume and NSS in groups IR,T,Y and NS were significantly higher than those in group C (P <0.05).The infarct volume and NSS in group T were significantly lower than those in group IR (P <0.05).TAK1 pro-tein of hippocampus in groups IR,Y and NS was significantly higher than that in group C (P <0.05).TAK1 protein of hippocampus in group T were significantly lower than that in group IR (P <0.05).LC3Ⅱ/LC3 Ⅰand Beclin1 protein of hippocampus in groups IR,T,Y and NS were signifi-cantly higher than those in group C,and the p62 protein of hippocampus in groups IR,T,Y and NS was significantly lower than that in group C (P <0.05).The LC3 Ⅱ/LC3 Ⅰand Beclin1 in group T were significantly lower than those in group IR,and the p62 protein of hippocampus in group T was significantly higher than that in group C (P < 0.05 ).Conclusion TAK1 mediated cell autophagy takes part in the mechanism of brain ischemia-reperfusion injury in rats.

ABSTRACT

Objective:To evaluate influence of electroacupuncture combined point massage on aptients with cartid ar- tery arteriosclerosiss.Methods:A total of 106 patients with carotid atherosclerosis were randomly divided into rou- tine group (n=54,received health education based on routine therapy)and electroacupuncture+ massage group (n=52,received additional electroacupuncture and point massage based on routine group).Both groups were treated for two courses,20 times/course, 1 time/d.Carotid intima-media thickness (IMT)and thickness of carotid plaques were measured and compared between two groups before and after treatment.Results:There were no sig- nificant difference in thickness of carotid plaques and IMT between two groups before treatment,P>0.05 all. Compared with before treatment,after treatment,there were significant reductions in thickness of carotid plaques [left:(0.104±0.103)mm vs.(0.044±0.031)mm,right:(0.111±0.093)mm vs.(0.055±0.046)mm]in elec- troacupuncture + massage group,and they were significantly thinner than those of routine group [left:(0.190± 0.175)mm,right:(0.223±0.183)mm],P<0.01 all;significant reductions in carotid IMT [left:(0.096±0.021) mm vs.(0.086±0.019)mm,right:(0.091±0.019)mm vs.(0.087±0.018)]in electroacupuncture + massage group,and they were significantly thinner than those of routine group [left:(0.105±0.016)mm,right:(0.103± 0.020)mm],P<0.05 or <0.01.Conclusion:Electroacupuncture combined point massage can effectively reduce thickness of carotid plaques and carotid intima-media thickness.
